  • [Title] Systemic Inflammation in Cachexia - Is Tumor Cytokine Expression Profile the Culprit?
  • This multifactorial syndrome resulting in involuntary and continuous weight loss is accompanied by systemic inflammation and immune cell infiltration in various tissues.
  • Understanding the interactions among tumor, immune cells, and peripheral tissues could help attenuating systemic inflammation.
  • Therefore, we investigated inflammation in the subcutaneous adipose tissue and in the tumor, in weight stable and cachectic cancer patients with same diagnosis, in order to establish correlations between tumor microenvironment and secretory pattern with adipose tissue and systemic inflammation.
  • Infiltrating monocyte phenotypes of subcutaneous and tumor vascular-stromal fraction were identified by flow cytometry.
  • Subcutaneous vascular-stromal fraction exhibited no differences in regard to macrophage subtypes, while in the tumor, the percentage of M2 macrophages was decreased in the cachectic patients, in comparison to weight-stable counterparts.
  • CCL3, CCL4, and IL-1β expression was higher in the adipose tissue and tumor tissue in the cachectic group.
  • In both tissues, chemotactic factors were positively correlated with IL-1β.
  • Furthermore, positive correlations were found for the content of chemoattractants and cytokines in the tumor and adipose tissue.
  • The results strongly suggest that the crosstalk between the tumor and peripheral tissues is more pronounced in cachectic patients, compared to weight-stable patients with the same tumor diagnosis.

  • OBJECTIVE: The purpose of this study was to investigate the expression of human adipose tissue protein 53 (p53) in subjects who varied widely in terms of obesity and insulin resistance.
  • METHODS: p53 was analyzed in human adipose and isolated adipocytes, in high fat-fed and GLP-1R KO mice, during in vitro adipogenesis, and in adipocytes after high glucose, rosiglitazone and inflammatory conditions.
  • Accordingly, the improvement of insulin sensitivity with surgery-induced weight loss (+51%, P=0.01) and metformin (+42%, P=0.02) led to increased adipose p53.
  • CONCLUSION: Inflammation and insulin resistance exert dual effects on adipose p53, which seems to be the final result of these opposing forces.

  • Although adipose tissue-derived estrogen contributes to obesity-associated risk for estrogen receptor (ER)-positive breast cancer, the estrogen-independent impact of adipose tissue on tumor invasion and progression needs to be elucidated.
  • Here, we show that adipose stromal cells (ASCs) significantly stimulate migration and invasion of ER-negative breast cancer cells in vitro and tumor invasion in a co-transplant xenograft mouse model.
  • Our study also identifies cofilin-1, a known regulator of actin dynamics, as a determinant of the tumor-promoting activity of ASCs.
  • Depletion of IL-6 from the ASC-conditioned medium abrogated the stimulatory effect of ASCs on the migration and invasion of breast tumor cells.

  • [Title] Gene expression differences in adipose tissue associated with breast tumorigenesis.
  • Long thought to function only as an inert energy storage depot, the role of adipose tissue in breast tumorigenesis has been largely ignored.
  • In light of increasing rates of obesity and use of breast conserving therapy and autologous fat grafting, improved understanding of the role of adipose tissue in tumor etiology is crucial.
  • Thus, adipose tissue adjacent to and distant from invasive breast tumors (n = 20), or adjacent to non-malignant diagnoses (n = 20) was laser microdissected from post-menopausal women.
  • Gene expression data were generated using microarrays and data analyzed to identify significant patterns of differential expression between adipose tissue groups at the individual gene and molecular pathway level.
  • Pathway analysis revealed significant differences in immune response between non-malignant, distant, and tumor-adjacent adipose tissue, with the highest response in tumor-adjacent and lowest in non-malignant adipose tissue.
  • Adipose tissue from invasive breasts exhibits increased expression of anti-inflammatory genes such as MARCO and VSIG4 while genes differentially expressed between tumor-adjacent and distant adipose tissue such as SPP1, RRM2, and MMP9, are associated with increased cellular proliferation, invasion, and angiogenesis.
  • These data suggest that molecular profiles of adipose tissue differ depending on presence of or proximity to tumor cells.
  • Heightened immunotolerance in adipose tissue from invasive breasts provides a microenvironment favorable to tumorigenesis.
  • In addition, tumor-adjacent adipose tissue demonstrates expression of genes associated with tumor growth and progression.
  • Thus, adipose tissue is not an inert component of the breast microenvironment but plays an active role in tumorigenesis.

  • [Title] Well-differentiated liposarcoma/atypical lipomatous tumor of the oral cavity: report of three cases and review of the literature.
  • Atypical Lipomatous Tumor/Well Differentiated Liposarcoma (ALT/WDLS) is a soft tissue sarcoma of intermediate malignant behavior, most frequently affecting the retroperitoneum and lower extremities.
  • Oral liposarcomas are very rare neoplasms, the most common histological subtype being ALT/WDLS.
  • Analysis of the salient clinicopathologic features of 63 oral ALT/WDLS cases previously reported in the English language literature, as well as of the 3 cases presented in this study, indicates that the indolent biologic behavior of this tumor justifies its designation as a locally spreading malignant neoplasm, affording a rather conservative surgical approach.

  • Pleomorphic lipoma is a relatively rare adipocytic neoplasm, occurring predominantly in elderly males in the subcutaneous tissues of the neck or shoulder.
  • We hereby report a case of a 60-year-old female patient, presenting with an intramuscular, posterior shoulder mass.
  • CD34 staining by immunohistochemistry further supported the diagnosis.
  • Differential diagnosis and the cytological diagnostic pitfalls of pleomorphic lipomas have been discussed with a review of the literature.

  • [Title] Lipomatous tumours of the head and neck: a spectrum of biological behaviour.
  • Lipomatous tumours are rare in the head and neck region.
  • Each lipomatous tumour has to be treated accordingly.
  • Only in small (<5 cm), superficial soft tissue tumours or when magnetic resonance imaging has demonstrated specific features of lipoma, may diagnostic biopsy be omitted.
  • Adequate preoperative diagnosis is important to assure adequate tumour control as well as optimal functional and cosmetic outcome.
  • The major problem in the treatment of lipomatous tumours of the head and neck region is the presence of nearby delicate structures.

  • In human and animal models of cancer, body composition assessment and morphological analysis reveals adipose atrophy and presence of smaller adipocytes.
  • Increased lipolysis and fat oxidation, decreased lipogenesis, impaired lipid depositionand adipogenesis, as well as browning of white adipose tissue may underlie adipose atrophy in cancer.
  • Inflammatory cytokines such as interleukin-6 (IL-6), tumor necrosis factor alpha (TNF-α), and interleukin-1 beta (IL-1β) produced by the tumor or adipose tissue may also contribute to adipose depletion.
  • Identifying the mechanisms and time course of fat mass changes in cancer may help identify individuals at risk of adipose depletion and define interventions to circumvent wasting.
  • This review outlines current knowledge of fat mass in cancer and illustrates the need for further studies to assess alterations in visceral and subcutaneous adipose depots and possible mechanisms for loss of fat during cancer progression.
